Tibial Tubercle Osteotomy Procedure

Tibial tubercle osteotomy is a surgical procedure commonly used in the treatment of patellar instability, osteoarthritis, and anterior knee pain. During this procedure, an orthopedic surgeon will reposition the tibial tubercle to correct misalignment and improve the way that the patella tracks. The tibial tubercle is a bony prominence on the front of the shinbone where the patellar tendon attaches the quadriceps muscles to the leg. Tibial tubercle osteotomy has been shown to effectively restore proper alignment of the extensor mechanism, leading to improved stability and decreased pain during activities that stress the patellofemoral joint.

When is this Procedure Necessary?

A tibial tubercle osteotomy is considered medically necessary when conservative treatments have failed to alleviate symptoms of patellar instability or arthritis-related pain. Patients may be a candidate for this procedure if:

Patients may be candidates for this procedure if they experience the following symptoms:

  • Their knee repetitively dislocates.
  • Osteoarthritis causes relentless pain.
  • Physical therapy and other conservative efforts were ineffective.

What To Expect

Patients will be under general anesthesia to ensure their comfort throughout the procedure. The surgeon will make an incision to use an arthroscope to view the region. Next, the surgeon will make an incision at the front of the leg below the patella. The surgeon will then reposition the tibial tubercle and attach it using two screws. When the goal of the procedure has been achieved, the surgeon will close the incision.

Following the procedure, individuals should anticipate a period of immobilization using crutches and wearing a brace or cast for several weeks to promote healing. Physical therapy will play a critical role in rehabilitation by gradually introducing weight-bearing exercises and range-of-motion activities.
