Lapidus Bone Fusion

Lapidus bone fusion is a surgical procedure that fuses the first tarsometatarsal (TMT) joint, located between the first metatarsal and the medial cuneiform bones in the midfoot. This procedure is primarily used to correct hallux valgus deformity (bunion) associated with hypermobility or instability of the first ray. During the surgery, the cartilage surfaces of the first TMT joint are removed to expose raw bone, and the joint is realigned to restore proper foot mechanics. Internal fixation, such as screws or plates, is applied to stabilize the bones and facilitate solid fusion. By permanently joining the bones of this joint, the Lapidus fusion eliminates abnormal motion that contributes to deformity and pain, leading to improved alignment, stability, and function of the foot. Postoperatively, the foot is immobilized, and weightbearing is gradually resumed as the fusion consolidates over several weeks.

As with most surgical procedures, it is highly recommended that patients first consider conservative treatment options. Although you cannot treat underlying arthritis, it is possible to find ways to manage your pain. Often, patients find relief from conservative options including physical therapy, hot and cold therapy, and anti-inflammatory pain medications. 

If these methods are ineffective, you may begin to consider surgery. Depending on your condition, your surgeon will recommend the most effective, least invasive treatment path. Patients are placed under general or regional anesthesia. The surgeon makes an incision along the inside of the foot to access the joint between the first metatarsal bone and the medial cuneiform bone. Damaged cartilage is removed, and the bones are positioned for proper alignment. Screws or plates are placed to hold the bones together until fusion occurs. The incision is then closed, and the foot is placed in a cast or boot. After surgery, patients typically cannot bear weight on the foot for several weeks. Swelling and soreness are common but improve over time. A gradual return to activity is expected once the fusion is confirmed to be solid on X-rays.
