Open Rotator Cuff Repair Procedure

Open rotator cuff repair is a surgical procedure to fix a damaged tendon or muscle in the shoulder joint that is causing shoulder pain and/or decreased range of motion. Depending on the severity of the tear, there are different ways an orthopedic surgeon might approach rotator cuff surgery. An open repair is typically the ideal option for individuals who have suffered a large tear to the rotator cuff. If the tear is smaller and less severe, however, patients may choose an arthroscopic approach instead.

When left untreated, a rotator cuff tear can cause painful and debilitating symptoms. In some cases, the condition can worsen with time if patients continue to engage in strenuous activities that put pressure on the shoulder joint. Patients may consider an open rotator cuff repair surgery if they have been diagnosed with a torn rotator cuff and it is not responding to conservative treatments. Oftentimes, patients with a torn rotator cuff can find relief through therapies such as physical therapy, hot/cold therapy, and avoiding activities that involve heavy mobilization of the shoulder joint. 

An open rotator cuff repair procedure may be necessary if:

  • The patient has suffered a large tear to a muscle or tendon in the shoulder joint
  • The patient is experiencing debilitating shoulder pain
  • The patient’s injury is not responding to conservative treatments
  • The patient’s condition would not be effectively treated using an arthroscopic rotator cuff procedure

During an open rotator cuff repair procedure, the patient will be under general anesthesia to ensure their comfort throughout. The surgeon will begin by making an incision to access the torn tendon. The surgeon will then reattach the tendons to the bone using sutures or anchors. If necessary, the surgeon will also remove any additional damaged bone tissue or fragments that are causing pain.
 
This is considered an invasive procedure since it requires a large incision. Therefore, the recovery time will be longer than an arthroscopically performed arthroscopically. Most patients will experience a full recovery at around 6 months, however, the joint will begin healing in the weeks following. Patients may experience pain at the surgical site in the week following the procedure, and will likely be given pain medications to manage any discomfort. Many patients will attend physical therapy to ensure the shoulder joint is healing and progressing with time.
